Important Tax Information – Form 1095-C

In accordance with IRS regulations, Silver Star will not automatically distribute Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, to employees.

Employees and former employees who would like a copy of their Form 1095-C may request one at no charge by contacting:

Requests must include the individual’s full name, current mailing address, Social Security number (or last four digits), and the tax year requested.
